Summary

Welcome to the Subject Access Request Portal for the University Hospitals of Derby and Burton NHS Foundation Trust. 

A Subject Access Request (SAR) is a request to an organisation (under the Data Protection Act 2018) for a copy of personal data held by that organisation.

To ensure confidentiality you will need to supply identification as part of the request process. You can find out what you need to supply by downloading the Proof of Identification Document (PDF).

How to make a request

If you would like access to your health records you can request them digitally through our secure SAR portal.

When making your request, please ensure that correct application type is selected.  This will enable the team to process your request effectively.

You can get a copy of your health records through this online portal by creating an account via this link, https://uhdb.ams-sar.com/Account/SignUp or via the 'Don't have an account? Sign up' button at the bottom of the page.  

If you are an organisation or representative, you should also use this online portal.
  
If you are applying to access the records of more than one person you must complete a separate application for each patient.

This portal is to be used to request Health Records and Imaging from the University Hospitals of Derby & Burton NHS Foundation Trust.

When making a Subject Access Request please ensure you have a form of ID proof of address and consent if applicable, ready before submitting your request and that you select the application form which matches the information you are asking to receive.
